To be Grim Reaper ready ten, free to download, forms need to be completed before your demise.  This will make it less stressful for the person handling the registration of your death. 

The information required to complete these 10 forms include –

1. Your doctor’s name and contact address for a medical certificate of death to be issued.

2. Your first, middle, surname, previous/maiden name.  Proof of postal address, proof of marital status, birth certificate, place of birth.  Surviving spouse name & NI number, death certificate of spouse if already widowed.  Date and place of death.  Present, last, previous occupation and pension details.  Spouse or father’s occupation.  NHS number, organ donor + card, religion details.  Address of death registration office of where you live. 

3. Details of how you want your funeral arranged. 

4. Executor’s name, address and contact details.  Details and documents of passports, driving licence, registered vehicles, Blue badge, Council tax bill. 

5. Eligibility for bereavement benefits. 

6. Details and proof of benefits and pensions received, taxes paid and businesses owned. 

7. Details and locations of wills, property owned, cash, bonds and shares. 

8. Name and contact details of family and friends. 

9. How to handle family heirlooms, jewellery, credit cards, loan agreements, insurance policies, unpaid bills, pets, memberships of various clubs/societies, power of attorney. 

10. Checklist for the person handling the death registration.
